Puke Whakaae Whakaahua – Individual Consent Form

Māui Studios Aotearoa is seeking your consent to use your image, audio or video record as explained below.

Your consent is voluntary. You can choose whether you provide your consent or not.

If you give your consent, you are granting Māui Studios Aotearoa, the entity photographing or recording you, and its licensees and assignees full permission to license and/or use any photographs, film or recording (“Works”) taken of you (including your appearance, likeness and form), for any purpose whatsoever (including, but not limited to, this project, promotional purposes, publications, website, intranet, Facebook and other social media).

The Works may be cropped, altered, transformed or reproduced in any way, in any current or future media (including, but not limited to, print, TV, film, digital and the internet), and may also be combined with any other works or text.

You’re agreeing that you have no rights or copyright in the Works (and any derivative works) and hereby release the Māui Studios Aotearoa and its licensees and assignees from any and all claims and demands in connection with the Works or their future use.

All images, audio or video records featuring you will be held by Māui Studios in a secure image library. Where your personal information can be readily retrieved, you can request access to records, and for corrections to be made to any personal details provided by you.

If at any time you wish to withdraw your consent, you may notify Māui Studios to that effect in writing by emailing Kiaora@mauistudios.co.nz. Withdrawing your consent will not affect any actions taken before the receipt of the written notification.
